The Healthy Start Care Coordinator is a clinical social work position providing care coordination, case management, education, psycho-social counseling and referral/follow-up to women during pregnancy and to their infants after delivery and up to the age of three years. The Healthy Start Care Coordinator exercises independent judgment and works with a minimum of supervision. Work may be performed in a variety of settings including office and home settings.
Minimum Job Qualifications:
Minimum education High school graduate with 5 years work experience in working with families, social work, education, child development and/or parenting education.
Must have a valid driver’s license, a good driving record and be insurable under the corporate policy.
Provides care coordination to Healthy Start clients and families.
Conducts case management, including home visits and referral and follow-up, following Healthy Start guidelines.
Provides home visits to other pregnant or parenting families as indicated.
Maintains client records according to standards.
Assists in collection of statistical data and evaluation of information to assure objectives are met and standards are maintained.
Provides parent education and affirmation in prenatal health, parenting skills, and infant/child health with referral to other providers as indicated.
Conducts organized education class sessions as needed.
Provides assessment, support, education and intervention for Healthy Start and other identified high risk clients.
Provides a structured interaction between the professional and client which assists client to identify concerns or needs by addressing social, emotional and behavioral, and environmental issues that affect the client’s well-being.
Develops a family support plan to meet identified needs.
Documents all activities appropriately in the medical record and codes provision of all services accurately into the WELL FAMILIES system.
